Dear Parent/Guardian, 

Thank you for allowing your child to participate as a volunteer at Reggae Genealogy, organized by Island Society for the Promotion of Artistic and Cultural Education (Island SPACE). In order to ensure the safety and well-being of all participants, we kindly request your consent by reviewing and signing this consent form:

Parent/Guardian Consent: 

I, the undersigned parent/guardian of the named volunteer, give my consent for them to participate as a volunteer at an event organized by Island SPACE. I understand that my child may be volunteering at an indoor or outdoor event, and I acknowledge the inherent risks associated with related activities. 

Medical Information: 

I hereby affirm that my child is in good health and able to participate in the volunteer activities. In case of emergency, I authorize Island SPACE to seek medical attention for my child. 

Release and Waiver: 

I, the undersigned, release Island SPACE, its organizers, volunteers, and affiliated personnel from any liability for injuries, damages, or losses that may occur during the event. 

Photography and Publicity: 

I grant permission for photographs or videos of my child taken during the event to be used by Island SPACE for promotional and publicity purposes. 

Parent/Guardian Signature: 

I have read and understand the terms and conditions outlined in this consent form, and I willingly grant permission for my child to participate in the related Island SPACE event / activity.
